Medical (D4)

The fitness-to-drive assessment on the DVLA D4 form — when to book it so it's still valid when you apply, and how to avoid an incomplete form.

Timing: Book once DBS and training are close — it has a four-month validity.

What it is

A medical examination to Group 2 (bus and lorry) standards, recorded on the DVLA D4 form by a doctor who can access your GP records. The council currently states the completed D4 medical must be dated within four months of your application date, with all eight pages complete. [A]

How it works

  1. 1Download the council's current medical certificate template from the 'Downloads' section of its application page, together with the D4 form.
  2. 2Book with your GP or a medical provider who can access your full GP records. Prices vary — compare before you book.
  3. 3Check every one of the eight D4 pages is complete, with your name and date of birth on each page.
  4. 4Upload it with your application. If the upload fails, the council asks you to email it with your WTP reference once you've submitted.

Common mistakes

  • Booking the medical too early, so it's more than four months old when you apply.
  • Pages left blank, or name/date of birth missing from some pages.
  • Using a provider who cannot access your GP records.
